Receive your certificate
Once the doctor is satisfied you meet the fitness criteria, your Form 1A is signed, stamped with the doctor's registration details, and sent to you digitally as a PDF — ready to upload to the document section of your Sarathi application, or to carry to your Punjab RTO/SDM office as required.

If the Doctor Needs More Information
Sometimes a single consultation isn't enough to complete a fair assessment — for example, if you've reported a vision or hearing issue that needs supporting documentation.

If a Certificate Can't Be Issued
Form 1A exists to confirm you're genuinely fit to drive, so every application is assessed on its own medical merits — not guaranteed in advance.
A certificate won't be issued if the consulting doctor determines that your current medical condition means you don't meet the fitness standard (for instance, an uncontrolled seizure disorder or vision that isn't correctable to the required standard).

Under India's telemedicine guidelines, a registered doctor can conduct a valid consultation remotely and issue a signed medical certificate. How do I actually submit this to my RTO?
You upload the signed PDF yourself, under the medical certificate/document section of your Sarathi application, or hand a printed copy to your RTO or SDM office if you're applying offline. We don't upload it to Sarathi on your behalf — the certificate is yours to submit.
I wear glasses / have diabetes / have mild hearing loss. Can I still get certified?
In most cases, yes — the doctor evaluates your overall fitness, and conditions that are well-managed or corrected (like vision corrected with glasses) are routinely certified fit, often with that correction noted on the form. It's the doctor's clinical call after reviewing your case.
Is this valid for every RTO in Punjab?
