Form 1 is a self-declaration
You fill it out yourself, stating you have no disqualifying medical condition. No doctor is involved.
Form 1A is a doctor's certification
A registered medical practitioner examines you and certifies your fitness. This is what's required once you're 40 or older, or applying for a transport/commercial licence.
What Does the Doctor Actually Check?
Form 1A isn't a rubber stamp — it's a genuine, if quick, medical assessment.
Vision
Distance vision and, where relevant, field of vision. Wearing glasses or contact lenses is fine; the doctor notes if correction is used.
Colour vision
The ability to distinguish traffic signal colours.
Hearing
General hearing adequacy; a hearing aid, if you use one, doesn't disqualify you on its own.
Limb and mobility function
Whether any physical limitation would affect your ability to safely operate a vehicle.
Conditions affecting consciousness or safety
Including epilepsy with recent uncontrolled seizures, fainting or blackout episodes, or any condition that could impair judgement or alertness while driving.
The form also has an optional field for blood group and Rh factor, which you can choose to have noted on your licence — useful in an emergency, though it's entirely optional.
Karnataka's RTO and Portal Setup
Karnataka runs driving licence services through Sarathi, layered with the state's own Seva Sindhu citizen services portal and Karnataka One service centers for those who prefer assistance in person. The state has more than 68 RTOs, and Bengaluru alone is served by 8 of them — including RTO codes KA01, KA03, KA04, KA05, KA41, KA51, KA52, and KA53.
Because Form 1A is a central government format under the Motor Vehicles Rules, none of this affects which certificate you need or how it's used — it's identical whichever Karnataka RTO your application sits with.

Is Form 1A different from the Form 1 I might have filled out before?
Yes — Form 1 is a self-declaration you complete yourself; Form 1A requires a doctor's examination and signature. If your Sarathi application specifically asks for Form 1A, only the doctor-certified version will satisfy it.
Does it matter which of Bengaluru's 8 RTOs my application is with?
No. Form 1A is a standard central government format, valid the same way across all of them — KA01 through KA53 and everywhere else in Karnataka.
I wear glasses or have diabetes — can I still get certified?
In most cases, yes. The doctor evaluates your overall fitness, and well-managed or corrected conditions are routinely certified fit, often with the correction noted on the form.
Will an online consultation be accepted, or do I need an in-person exam?
A registered doctor can conduct a valid consultation remotely under India's telemedicine guidelines and issue a signed certificate. What matters to your RTO is the standard Form 1A format with a genuine doctor's registration number and signature.
How do I submit this to Sarathi or Seva Sindhu?
You upload the signed PDF yourself, in the document section of your application, or hand a printed copy to your RTO if applying offline. We don't submit it on your behalf.
Should I add my blood group to the form?
It's optional. Some people choose to, since it can be useful in an emergency; it has no bearing on whether your certificate is approved.
How long does the process take?
