NCR Spans Three States — Does That Change Anything?

Not for the certificate itself. Form 1A is prescribed under Rule 5 of the Central Motor Vehicles Rules, 1989, a central regulation — so it's identical whether your application is with Delhi, Haryana, or Uttar Pradesh's transport department. What does change across the three is which RTO and portal you're actually dealing with:

Delhi

NCT of Delhi. RTO codes vary by district.

Haryana

Gurugram and Faridabad. Gurugram: HR-26; Faridabad: HR-51 (some sub-offices use other codes).

Uttar Pradesh

Noida and Ghaziabad. Noida: UP-16; Ghaziabad: UP-14.

A common point of confusion: your driving licence or vehicle registration state doesn't have to match where you currently live or work. If you moved from Ghaziabad to Gurugram, for instance, your Form 1A application still goes through whichever state your Sarathi application is actually registered under, not your current address.
